The fix for me, was switching from Vulkan to Direct X11 on the Larian screen, before loading BG3 proper. Switching to Direct X11 actually resolved about 60% of my latency issues as well. Fair warning. Game looks substantially uglier using Direct X11. However ugly and playable is far, far superior to gorgeous and a digital paper weight.
